About This Registry

This page lists formal sanctions issued by the ICONIC Board Ethics & Standards Committee. Only Level 3 (Public Reprimand) and above appear in this public registry. Lower-level private and educational sanctions are not published, in accordance with Board policy.

What is Published

Level 3 Public Reprimand, Level 4 Probation, Level 5 Suspension, and Level 6 Revocation sanctions are listed here by name, credential ID (partially masked), sanction type, and effective date.

What is Not Published

Level 1 (Advisory Letter) and Level 2 (Private Written Reprimand) sanctions remain confidential between the practitioner and the Committee. Not all complaints result in formal sanctions.

Record Retention Policy

Public records remain in this registry for the durations listed below. Upon expiry, records are removed from the public register. Records may also be removed earlier if successfully overturned on appeal.

Public Reprimand 3 years from date of issue Automatically removed from the public register after 3 years, provided no additional sanctions are issued during that period.
Probationary Status Duration of probation + 2 years Record remains public for the full probationary period and for 2 years following its successful completion or termination.
Suspension Duration of suspension + 3 years Record remains public for the duration of the suspension and for 3 years following reinstatement or lapse of credential.
Revocation Permanent Revocations are maintained in the public record indefinitely unless overturned on successful appeal, at which point the record is amended to reflect the appeal outcome.

Frequently Asked Questions

Common questions about the Public Disciplinary Records registry.

There are several reasons a sanction may not appear in this registry. First, not every complaint results in a formal sanction — the Ethics & Standards Committee may determine that the conduct did not constitute a violation, that the concern falls outside the Board's jurisdiction, or that an educational or private resolution was more appropriate. Second, sanctions at Level 1 (Advisory Letter) and Level 2 (Private Written Reprimand) are confidential and are never published publicly. Only Level 3 and above appear in this registry. If you submitted a complaint and have questions about the outcome, please contact ethics@iconic.health.

Practitioners subject to a formal sanction have the right to appeal the decision through the Board's formal appeals process. Appeals must be submitted within 30 days of the sanction notice. If an appeal is successful, the record will be amended or removed from this registry as appropriate. To initiate an appeal, refer to the Disciplinary Process & Appeals page, or contact ethics@iconic.health. Records cannot be disputed outside of the formal appeals process.

A public record may only be removed before the standard retention period expires in two circumstances: (1) the sanction is successfully overturned on appeal, in which case the record is removed or amended to reflect the outcome of the appeal; or (2) an administrative error is identified and corrected through the Board's internal review process. Requests for early removal that do not meet these criteria will not be granted. Records are automatically removed upon expiry of the applicable retention period without requiring any action from the practitioner.

This registry only shows disciplinary sanctions. To verify whether a practitioner currently holds an active ICONIC Board credential in good standing, use the Credential Verification tool. Credential verification reflects the current status as of the date of lookup. A practitioner may appear in this registry with a past public reprimand while still holding an active credential in good standing — these are independent records.

New sanctions are added to this registry following the conclusion of the formal disciplinary process, including any waiting period for appeals. The registry is reviewed and updated on a quarterly basis; however, new records may be added at any time following the completion of a case. The Transparency Report badge at the top of this page indicates the most recent scheduled update cycle.
